  3. Discipline and Guidance Policy (1099), 

  4. Emergency Preparedness Plan and Acknowledgement Form,  

  5. Safe Sleep Policy Form (J-800-2550), 

  6. Outdoor Activity Space Plan

  7. Potty Training Policy

  8. CACFP Meal Child Enrollment Forms

 

All enrollment forms must be submitted with a current copy of your child’s immunization records before your child begins care.

7:30am to 5:30pm

Yorktown Location:

7:00am to 5:30pm
